October 15 Extension Checklist: Don’t File Naked

Documents to gather before the deadline.

If you filed Form 4868 (or were granted an automatic extension for certain situations), October 15 is often the deadline to file your complete and accurate Form 1040 for the prior year. “Extension” only moved the filing date; it did not move your obligation to pay tax that was due in April. This checklist keeps you from filing a return that is missing the documents that trigger refunds or avoid notices.

  • Gather all W-2s and 1099s (including corrected/amended versions).
  • Download final brokerage realized gain/loss and 1099-B PDFs (not January drafts).
  • Reconcile estimated taxes and withholding against April and June payments.
  • File before October 15 if that is your deadline; confirm your state due date separately.

  1. W-2 from every employer (check for corrected W-2c if you had payroll fixes).
  2. 1099-NEC / 1099-K for gig and platform income.
  3. 1099-INT / 1099-DIV from banks and funds.
  4. 1099-B and supplemental equity statements for stock sales.
  5. SSA-1099 if you receive Social Security benefits.

Document proof callout: For wages, sum W-2 Box 1 and compare to what you intend to report on Line 1 of Form 1040 before you sign.

Investments and equity comp

  • RSU / ESPP: You need vest reports and sale confirmations. Basis issues are the #1 reason extended returns get amended later.
  • Crypto: Exchanges may issue 1099 forms with incomplete basis; export transaction history for the full year.

Deductions and credits (if itemizing or claiming credits)

  • 1098 mortgage interest, property tax bills (subject to limits).
  • 1098-T for education credits, 1095-A for Marketplace health coverage.
  • Childcare provider EIN or SSN and receipts for dependent care credit if applicable.

Payments already made

  • Federal estimated tax vouchers or EFTPS confirmations for April, June, September, and January (for the prior tax year cycle).
  • State payments if you pay state estimates separately.

Worked example: “I’m still waiting on a K-1”

Partnership K-1s can arrive late. If you are still missing a final K-1 near October:

  • Consider whether you qualify for additional extension rules beyond October 15 (certain situations apply to out-of-country taxpayers or specific forms; check current IRS guidance).
  • Do not guess K-1 Box 1 income; an amended return is cheaper than penalties.

Does October 15 apply to my state?
Often, but states differ. Verify your state’s extended due date.

Will I owe penalties if I owe tax?
Filing an extension does not remove failure-to-pay penalties from April if you underpaid. Use Form 2210 only if applicable to your facts.
